2022 Workers' Compensation Application Process


Before Starting the Application Process Review the Following

As of February 1, 2020, all New York State Workers' Compensation applications must be completed online.

NYS Workers' Compensation Board 

Workers' Comp will send Queens County your completed application to review and credential.

Paper applications will no longer be accepted and will be returned to the sender.


The applying physician will have to go through all three of the following steps:

    1. Create a profile and login.
    2. Complete approximately 2 hours worth of required online training.
    3. Once the above steps are done you can begin the application and upload the required documents listed below. Note: ALL documents have to be submitted online and you cannot leave an application in progress. If all of the required documents are not uploaded at the time you submit your application, your application will not be processed.

First, be sure that at least one (1) practice location is in Queens. If not, select the county where your office is located.

        • Copy of current curriculum vitae (CV)
        • Copy of current Board Registration(s) - If not Board Certified, proof of successful completion of a 3-year residency program
        • Two (2) current letters of recommendations - Must be on letterhead from Chief of Service of a hospital, or a comparable professional peer
        • Payment in the form of a check for $850.00, or call our office at 718.268.7300 to pay by credit card. Please make your check out to:

Applicants are eligible for ONE (1) YEAR Free Membership at the Medical Society of the County of Queens. If interested, contact our office before submitting payment. Membership is not required.

Workers' Comp processing fees are non-refundable regardless of outcome.

You will be contacted directly from Workers' Comp for additional document(s) if necessary.
